GDPR, Accident Waiver and COVID 19 Guidelines

All information collated is stored securely and appropriately ensuring confidentiality of information, under the General Data Protection Regulations (GDPR). All information collated is for the safety and participation screening purposes and to identify an emergency contact for each individual.

ACCIDENT WAIVER & RELEASE OF LIABILITY CONTRACT

I, the participant in Ladies That Tri training sessions , hereby certify that I have a sufficient level of fitness to participate in the training sessions that are prescribed to me and that I am physically fit and have not been advised otherwise by a qualified medical person. I am aware that running is a potentially hazardous activity in an uncontrolled open course environment and carry with them the potential for serious injury or death. I hereby agree to assume all risks associated with Ladies That Tri coaching training sessions and all activities incident thereto including, but not limited to: extreme physical exertion; falls; contact with other participants, event equipment, vehicular traffic, potentially dangerous animals; the effects of weather, road conditions and cold water.

I hereby assume all of the risks of completing Ladies That Tri coaching and training sessions that have been prescribed. I realise that liability may arise from negligence or carelessness on the part of the entities or persons named below or from dangerous or defective equipment controlled by them.

Having carefully read the above waiver and all risks being known and appreciated by me and in consideration of my participation in Ladies That Tri coaching prescribed training sessions and all activities incident thereto; I hereby take action for myself and anyone entitled to act on my behalf as follows:

  1. Waive, release and discharge from any and all liability for my death, disability, personal injury, property theft or actions of any kind however caused which may hereafter accrue to me the following entities or persons: Ladies That Tri coaching, Coaches, directors, employees, volunteers, representatives, agents and sponsors.

  2. Indemnify and hold harmless the entities or persons mentioned above from any and all liabilities or claims made as a result of participation in Ladies That Tri Coaching prescribed training sessions, whether caused by negligence or otherwise.

  3. I grant my permission to Ladies That Tri coaching and of its transferees and licensees, to utilise any photographs, video recordings and any other references or records of training which may depict, record, or refer to me for any purpose, including commercial use, by Ladies That Tri coaching.

 

I certify that I have read and agree to all of the terms and conditions stated on this form. I acknowledge that this accident waiver and release of liability contract will be used by Ladies That Tri coaching and that it will govern my actions and responsibilities when undertaking Ladies That Tri training sessions and shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.  I recognise and understand all the terms and conditions set between my Coach and myself and agree to follow all the guidelines set out.  I have read this entire contract and understand its content without exception.

Covid-19 information for LTT

  • All policies and procedures for LTT group sessions will be a adhered to from direction of British Triathlon Federation and Government Guidelines.

 

  • Carrine Green is appointed as the COVID-19 officer for Ladies That Tri coaching.

 

  • COVID-19 Risk assessment and action plan are in place for all Ladies That Tri group and 1:1 sessions. Copies of these are available on request.

 

  • Social distancing should be adhered to at all session, before during and after the group sessions. Distancing of 2 meters where possible or 1 meter plus where this can not be achieved.

 

  • All participants, officials, volunteers and spectators must undergo a self-assessment for any Covid-19 symptoms. No one should leave home to participate in any activity if they, or someone they live with, has any of the Covid-19 symptoms.

 

  • People who are symptomatic should self-isolate for 7 days and household members for 14 days as per NHS guidance. Anyone who is self-isolating should not attend an outdoor sports facility/activity. Consider the risks of increasing your physical training load too fast where it might have an impact on your immune system that puts your health at risk

 

  • If you attend a session and then start to show symptoms please contact head coach Carrine Green (ladiesthattri@gmail.com, 07886444947) so that the track and trace procedures can be adhered to.

 

Traveling to and from the session:

  • Use toilet facilities at home before you leave. Indoor facilities can not be provided at this time. 

  • Wash your hands with soap and water for at least 20 seconds before leaving home to go to the venue (or use an alcohol gel if washing hands is not possible). Information on hand hygiene is available here.

  • Arrive as close as possible to when you need to be at the venue and allow others to leave before you enter. If you need to wait, then do so away from the venue and clear of the gates.

  • Avoid touching gates, fences, benches, etc. if you can.

  • Ensure you leave the venue before the end of your allotted time so that it is empty for the next participant.

  • Arrive changed and ready to participate. Shower at home as any changing rooms will be closed.

  • Do not congregate with others and adhere to physical distancing rules. No extracurricular or social activity should take place.

  • Where venues or sessions allow, ensure you have booked in advance and make payment online.

 

Equipment and clothing:

  • The sharing of equipment (such as bikes, wetsuits, helmets, floats, etc) must be avoided where possible, particularly equipment used around the head and face. Where equipment is shared, it must be cleaned before another person uses it. 

  • Equipment will not be hired or shared during any of the LTT sessions.

 

NHS Test and Trace:

  • A register will be held by Carrine Green for each group session to support the NHS test and trace programme by collecting relevant contact information on participants so everyone can be contacted if someone falls ill with Covid-19.

 

Hygiene:

  • Increased hygiene measures will be in place at all times. An outdoor handwashing facility will be available at The Barn as well as hand sanitiser. Face masks are to be worn at the individual’s discretion but are not compulsory.

  • If you need to sneeze or cough, do so into a tissue or upper sleeve. Dispose of your tissue into an appropriate bin or place in a plastic bag and take home. • Wash your hands afterwards for 20 seconds. • Avoid touching your face and ensure to clean your hands with at least 60% alcohol gel when you finish participating. • At all times be aware of current social distancing guidelines for those that exercise together from separate households. • Follow the government hygiene regulations including regular hand washing for at least 20 seconds.

 

 

Administering First Aid:

  • As there is no access to indoor facilities allowed, consideration should be made to house first aid equipment externally. All first aid equipment must be updated appropriately for the Covid-19 pandemic and first aiders must have undertaken appropriate additional training.
